Student Registration

The online student registration form is now open! All students within the CNMI Public School System must be registered for the new school year (2024-2025).

Before you begin…

Please get scanned copies of the following documents:

Valid student health certificate

  • Check the certificate expiration date

  • You can contact CHCC at (670)234-8950 for more information

  • This is required by law! (PL 06-10). See requirement notice here.

Sample health certificate

Official student & family records

  • Student’s birth certificate

  • Parent/guardian ID

  • Custody documentation (if applicable)

  • Medical documentation (if applicable)

Valid student health insurance

  • Medicaid is acceptable as along as the card is not expired and the student’s name is on the card.

  • Student insurance from private providers (Staywell, Moylan’s, etc.) is acceptable.

Academic Records (if applicable)

  • For transfer students: an official transcript copy is required.

  • For incoming 9th graders: an official middle school promotion card is required.
